On-Campus Specialties

The Clinic is patient driven, providing more than 25 specialties on our Clinic campus required to treat the multitude of chronic health conditions faced by our patients. Many of these chronic conditions require dedicated equipment, which is why the Clinic expansion is so vital in offering specialized patient care.  For this, the Clinic has expanded and added many unique areas, such as full imaging, a complete dental suite, ophthalmology equipment, cardiology and gynecology services and more, to help care for our patients and minimize any risk of interruption of care.  This reduces our patients time away from work, allowing for more comprehensive care with less time constraints.  For any procedures we are unable to provide on-site, we work with local providers to help deliver those necessary services.  As we continue to grow, our hope is to relieve the pressures on our local providers and ensure full, uninterrupted care at the Clinic.

Education

Education is vital to comprehensive care, and it takes place in every facet of the Clinic.

The Van Domelen Education and Wellness Building is the center of activity for our patient-focused instruction. Our expanded classes, presentations and interactive patient activities continue to grow as we design programs to answer the need of our patients. As we enhance the education process, our patients continue to use the Clinic not just for care, but education on all aspects of their life. This includes participation in Healthy Lifestyles, Movement Class, Meditation Class, Diabetic Education and more. We require at-risk patients to participate to better understand and manage their chronic conditions.

Through one-on-one counseling our patient medication compliancy is 98% compared to the US average 54%. Compliancy rate is critically important given the multitude of chronic diseases our patients manage.

Care Messaging has made a direct impact through informational reminders regarding education, compliance, treatment, and others, as necessary to help our patients manage their chronic disease.

Lab

All of our patients are facing chronic diseases, many of which face 3 – 5 conditions. To maintain patients optimal health requires education, medicine, and scheduled lab tests to monitor their compliance with treatment. All new patients receive lab test orders to assess any number of 18 metabolic indicators related to the advancement of disease, and/or the efficacy of treatment they are receiving. Our patients must be tested regularly to measure and manage their medical regimen and compliance, in a physician-controlled environment. This ability to monitor patients with chronic illnesses during a scheduled Clinic appointment without having to refer them to an outside source is key to the success of their health management. This is especially important, since time away from work for medical appointments equates to lost wages for our patients at the Clinic.

Lab tests run during patients visits in 20 minutes or less.

Medications Icon

Medications

The Clinic delivers care to qualified patients facing chronic diseases, many of whom live with 3 – 5 chronic conditions. To maintain the highest quality of health possible, the Clinic provides all medications free of charge and patients receive their medications before they leave our campus. More than $6 million retail value is distributed at the Clinic in a year. Our standard protocol of one-on-one counseling, helps our patients maintain a compliancy rate of 98%, compared to the national compliancy rate of 54%. Our formulary of over 600 different medications is received through patient assistance directly from the manufacturer as well as through our partnerships with Americares, Direct Relief, and Dispensary of Hope.

SHOW YOUR SUPPORT

We rely on your support to deliver quality medical care to low income, working but uninsured Collier County adults, using a volunteer professional staff. The Clinic is funded solely by private philanthropy and accepts no government money.

The overwhelming generosity and support of our donors allows us to leverage every $1 donated into $7 of services for our patients.
