The Clinic is located at the corner of Central Avenue and Goodlette Road North.


To qualify as a patient at Neighborhood Health Clinic you must:

  • Be 19 to 64 years of age
  • Be a Collier County resident
  • Be working a minimum of 80 hours a month
  • Not be actively enrolled in health insurance, Medicaid, or Medicare

To receive specialty care at the Clinic you must be a qualified patient. Final determination for a patient to qualify at our facility is governed by the Board of Directors. Eligibility does not constitute availability of services.

The Clinic Does Not:

  • Provide care if you have access to health insurance through your employer, your spouse’s health insurance, Medicare, Medicaid, Social Security, Disability Insurance, Worker’s Compensation or Unemployment Benefits
  • Treat AIDS, tuberculosis, pregnancy or pregnancy-related conditions. These services are provided by the Collier County Department of Health.

What To Bring To Your Qualification Appointment

  • Photo ID – Your photo ID may be used for proof of Collier County residency if it includes your current address. If the ID does not list a Collier County address, such as a passports, please bring in a document with your name and current address such as a bank statement, electricity bill, etc.
  • Proof of employment – Please bring your most recent paystubs for all working family members AND if possible, your most recent tax return
    • If your/your family member’s pay check includes stubs:
      •  Stubs must be from within the last 30 days
      • 2 stubs if you get paid biweekly, 4 stubs if you get paid weekly
    • If your/your family member’s paycheck does not include a stub, or your stubs are unavailable:
      • A letter that states where you are employed with the average number of hours worked per week and your average income per week when working.

Patient Responsibilities

  • Schedule a visit – You are required to call ahead to schedule a visit, unless it was schedules at your last visit.
  • Keep scheduled visit – Or call to cancel. Any cancellation less than 24 hours before the scheduled visit will be considered a NO Show. Two (2) NO SHOWS may result in ineligibility of services.
  • Be patient – Neighborhood Health Clinic delivers quality medical care using a volunteer professional staff. Although every effort is made to properly staff each Clinic visit there are times when we may be short on volunteers or the doctors may run behind schedule. Your patience is appreciated.
  • Supply Necessary Documentation – You are required to provide identification and paystubs or tax return forms when requested.
  • Be Courteous – Absolutely NO BEHAVIORAL OR VERBAL ABUSE. The physicians, nurses, pharmacists and administration volunteer their time to help you. Abusive behavior and/or language will not be tolerated.
  • Follow Instructions – You must take all medications prescribed for you as directed; get medication refills before you run out; keep all scheduled referral appointments.

Our Services

Patient services at the Clinic are provided to care for your medical needs once you are qualified. We do not provide employment physicals or “well” patient exams. We are your medical home providing a wide variety of medical specialties to care for your chronic conditions, keeping your schedule in mind with all services on our campus.

For any specialty care that is not provided on our campus, we will make referrals and assist with creating appointments for you to see the appropriate physician to care for you, while you continue to be a patient of the Clinic.
